Before you submit the withdrawal form, please review the information below. These details affect tuition, scheduling, makeup lessons, and your student's weekly lesson time.
Submitting this form is required to stop future tuition charges. Verbal notice to a teacher or front desk staff member does not complete withdrawal.
If you only need to miss a few lessons, staying enrolled may be the better fit.
While enrolled, you may still be able to:
Once a student withdraws, their weekly time may be offered to another student. We cannot guarantee that the same teacher, day, or time will be available if you return.
If lessons are not currently working well, withdrawing may not be the only option.

CSM requires at least 15 days' notice to stop weekly lessons.
To stop lessons at the end of the current month and avoid tuition for the following month, this form must be received by the 15th.
If this form is received after the 15th, the student may still have one or two lessons scheduled in the following month. Tuition may be charged for those lessons because they fall within the required 15-day notice period.
Families are responsible for tuition during the notice period whether or not the student attends.
